Student's signature and government-issued ID
 is required.
Under provisions of the 
Family Education Rights and Privacy Act of 1974, transcripts may not be released without written consent of the student.

Place your transcript orders through Parchment, LLCPricing and instructions are provided on the Parchment website.

  • PLEASE use proper capitalization to prevent holds or delays in the processing of your order.
  • To check the status of your order, log into your Parchment account and click on the ORDERS tab in the upper right corner. You will be able to view and track orders placed from there.

Instructions

  1. Review pricing and instructions in the Transcript Request form
  2. Complete the form choose one of the options below:
    1. Bring it in person or mail to:

Important Notes

  • Only the first 2 transcript requests are free. All requests afterwards will be charged at the prices specified in the Transcript Request form.
  • In order to redeem your free copies, the request must be submitted directly to the Admissions & Records office.
  • A&R does not email or fax transcripts.
  • Mail Service
    All transcripts are sent regular 1st class mail through the U.S. Postal Service. We do not provide Express/Overnight mailing service. When mailing transcripts, we cannot assure that your transcript will reach its destination. Once the transcript leaves Chabot College, it is the responsibility of the U.S. Postal Service to deliver. Lost transcripts may result in having to request and pay for additional copies.
